If you have previously been under the care of a dermatologist or have been treated by me at my previous office, please request a copy of your dermatology records from that office. The dermatology records can be helpful in many ways, including the identification of treatments which were particularly beneficial or previously not well tolerated. A letter of request should be sent (by mail or fax, call to confirm receipt) to the address where you previously received your care. Ideally, this should be done prior to your appointment. You may request to have your records sent to you or directly to this office. Alternatively, you may pick up your records from the previous office.
Our office is open Monday through Saturday. Business hours from Monday through Friday are 9:00am to 4:30pm. Please call for details on early morning, evening and weekend appointment hours. Emergency appointments are also available.

Credit card payment - If you would like to pay by credit card, please inquire before your visit as to the types accepted for payment. We currently accept MasterCard, VISA, and Discover.

We participate with most insurance plans. Please notify us of your insurance plan when making an appointment and present your insurance information upon arrival to the office.
If your insurance plan requires you to have a referral, you must obtain the referral from your primary care physician PRIOR to your arrival at the office. Insurance plans which require a referral from the primary care physician will not allow us to treat you under your insurance without the proper referral.
If you have an insurance deductible, payment for visits is due until your deductible has been satisfied, at which time we will accept your insurance for services that are covered. All insurance co-payments are due at the time of your visit.
